2. Timing: Sweepstakes begins
at 12:00:01 a.m. Chicago time on August 1, 2009 and ends at 11:59:59
p.m. Chicago time on December 15, 2009.
3. How to Enter: There are two separate ways to enter the Sweepstakes:
1) subscribers to gametimedecisions.net will automatically receive one
entry per membership; 2) send a 3 x 5 index card with your name, address,
email address and daytime phone number in an envelope with proper postage
to TCF Attn: GTD Sweepstakes 875 N Michigan Suite 3522 Chicago, Illinois
60611. One entry per person or e-mail address. Mail in entries
must be postmarked by December 15, 2009 and received by December 22,
2009
Incomplete or illegible entries are ineligible. Sponsor is not responsible for lost, incomplete, illegible, late, misdirected; technical or mechanical malfunctions; failures or malfunctions of phones, phone lines or telephone systems; failures of electronic equipment, computer hardware or software; or inaccurate entry information, whether caused by equipment, programming used in this contest, human processing error or otherwise. Sponsor reserves the right, in its sole discretion, to void any and all entries of an entrant when a sponsor believes the entrant has attempted to tamper with or impair the administration, security, fairness or proper play of this contest or website or to be entering the Sweepstakes from a location outside the participating states. Sponsor will report fraud committed in connection with this contest to enforcement authorities.
4. Determination of Winners:: On or about January 1, 2010 potential winners will be drawn at random from among all eligible entries by Sponsor. The potential winner will be contacted by the email and/or phone. Attached to the winner notification email will be an Affidavit of Eligibility, Liability Release, and a Publicity Release. The potential winner who wishes to claim his or her prize must print out and sign these documents. Instructions on where to send the documents and claim the grand prize will be contained in the winner notification email. Sponsor reserves the right to disqualify any potential winner and deny them the grand prize if the potential winner does not meet the eligibility criteria listed in these rules, does not have the documents completed correctly, or can not provide proof of identification as required. Odds of winning depend on the number of eligible entries received.
All prizes will be awarded. Failure to comply with any of the foregoing may result in disqualification.
5. Prizes: The following prize will be awarded:
Roundtrip coach-class airfare for two (2) people to/from any city in the continental United States (48 contiguous states). Approximate retail value $1,000.
Prize is not transferrable. Sponsor may substitute a prize of equal or greater value. All taxes, fees and surcharges on prizes are the sole responsibility of winner, including completion of Form W-9 Tax.
All travel arrangements are subject to
air travel availability, additional holiday/blackout periods and other
restrictions. All expenses not stated are the responsibility of the
winner and guest. Winner and guest must have necessary passport
or other travel documents. Guests under 18 years of age must be accompanied
by parent or legal guardian. All travel must be booked
at least 21 days in advance of travel date and completed by September
1, 2010
6. Other Terms and Conditions: By entering either contest, the
winner agrees that the Sponsor and those acting under the Sponsor's
authority may use winner's name, for advertising and promotional purposes
related to this contest without further consideration or notice. By
submitting their email addresses, all participants are granting permission
to be contacted by Sponsor via email regarding information and offers
related to their account. Participants will be given the opportunity
to “opt out” (or unsubscribe) each time they receive an email. Sponsor
is not liable for any printing or production errors associated with
the contest. BY ACCEPTING THE PRIZE, THE WINNER AGREES THAT
THE SPONSOR, ITS PARENTS, SUBSIDIARIES, AFFILIATES, ADVERTISING AND
PROMOTION AGENCIES, AND ALL OF THE SPONSOR'S OFFICERS, DIRECTORS, EMPLOYEES,
REPRESENTATIVES, DISTRIBUTORS, AND AGENTS, WILL HAVE NO LIABILITY WHATSOEVER
FOR, AND WILL BE HELD HARMLESS BY WINNER FOR ANY LIABILITY FOR ANY INJURY,
LOSS OR DAMAGES OF ANY KIND TO PERSONS, INCLUDING DEATH, AND
PROPERTY, DUE IN WHOLE OR IN PART, DIRECTLY OR INDIRECTLY, FROM THE
ACCEPTANCE, POSSESSION, USE OR MISUSE OF THE PRIZE OR PARTICIPATION
IN THIS CONTEST OR PARTICIPATION IN ANY CONTEST OR PRIZE RELATED ACTIVITY.
Sweepstakes intended for participation in the U.S. only and shall be
construed only according to U.S. law. Entrants agree the contests
are governed by Illinois law. Subject to all federal, state and
local laws. Void where prohibited by law.
7. Miscellaneous: In the event of a dispute, entries made by Internet will be deemed made by the authorized account holder of the e-mail address submitted at the time of entry. The "authorized account holder" is deemed the natural person who is assigned to an e-mail address by an Internet access provider, service provider or other online organization that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. A potential winner may be requested to provide Sponsor with proof that the potential winner is the authorized account holder of the e-mail address associated with the winning entry. If for any reason the Sweepstakes is not capable of running as planned, including due to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure, human error or any other causes beyond the control of Sponsor that corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Sweepstakes, Sponsor reserves the right in its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the Sweepstakes. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. Entrants agree that in the event of a dispute, all Sponsor’s decisions are final. The Sponsor is not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any e-mail or entry to be received by Sponsor on account of technical problems or traffic congestion on the Internet or at any web site, any combination thereof, or otherwise, including any injury or damage to entrant's or any other person's computer or mobile phone related to or resulting from participation or downloading any materials in the Sweepstakes. Sponsor is not responsible for late, lost, illegible, incomplete, stolen, or misdirected entries. CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEB SITE OR TELEPHONE NETWORK OR UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.
